How to Apply
An 'Application for Assistance' form must be fully completed and returned to the Tribunal for registration. The application must set out circumstances of the act of violence including:
- date, time and place
- what classification of victim the applicant is claiming as
- nature of injuries or cause of death
- whether matter was reported to police
- whether criminal proceedings arising out of the act of violence have been commenced
- amount and type of assistance sought
- whether the applicant has made any other applications for assistance in respect of the same act of violence
- whether the applicant has applied for compensation, damages or payments of any kind from any other scheme
- details of any relevant insurance cover
- whether the applicant elects to attend a hearing or have the application determined without a hearing.
Related Victims must also give the details of every other person whom they believe may allege they are a related victim of the act of violence or who may apply for funeral expenses through the Tribunal.
The Tribunal will also require:
- a statutory declaration to be completed verifying the contents of the application are true
- an authority for the Tribunal to obtain any other relevant information to enable it to determine the application.
If the application is not fully completed, it will be returned to you. Applicants may need to write a short explanation if they are unable to answer particular sections of the application applicable to them. An Information Guide is available to assist in completing the application.
