Title Summary
Application for Assistance Use this form when applying for assistance to VOCAT. The Information Guide will assist you in completing the form.
Application for Counselling - Form 4 PD1-2008 Applicant and counsellor much complete this form to request VOCAT to provide assistance for counselling sessions
Application for Extension of Time - PD6-2008 Use this form to apply to VOCAT for an extension of time in which to lodge an application.
Application to Access Information - Form 1 PD9-2008 All applications to access information about an application for assistance and/or documents contained in a Tribunal file must be made in writing using this form.
Assessment Report by non-treating practitioner -Form 1 PD2-2008 A report in this format is to be provided to the Victims of Crime Assistance Tribunal when an applicant or their legal representative has requested that a psychologist or psychiatrist prepare an assessment report for the Tribunal, and the psychologist/psychiatrist does not propose to provide counselling/treatment to the applicant; and the Tribunal has requested the preparation of an assessment report.
Counselling and Report Fee Invoice - Form 5 PD1-2008 Counsellors must complete this form in order to obtain payment of fees from the Victims of Crime Assistance Tribunal.
Dependency Claim Form - PD8-2008 Use this form in order to make a claim for dependency
Initial Report - More than 5 hours counselling - Form 2 PD1-2008 A report in this format is to be provided to the Victims of Crime Assistance Tribunal when the counsellor recommends that the applicant should have more than five hours of counselling.
Initial Report - Up to 5 hours counselling - Form 1 PD1-2008 A report in this format is to be provided to the Victims of Crime Assistance Tribunal when the counsellor has provided up to five hours of counselling to an applicant, and either the applicant is not seeking an award for further counselling, or the counsellor does not recommend more than five hours of counselling.
Loss of Earnings Claim Form - PD5-2008 Use this form in order to claim for loss of earnings.

Statutory Declaration Statutory Declaration form for use when required by the Victims of Crime Assistance Tribunal
Subsequent Report - Additional hours of counselling - Form 3 PD1-2008 A report in this format is to be provided to the Victims of Crime Assistance Tribunal when the counsellor recommends that the applicant should have further counselling beyond that previously awarded by the Tribunal.
Undertaking (Access Information) - Form 2 PD9-2008 All persons (other than media representatives) requesting access to information and/or documents contained in a Tribunal file must sign an undertaking in this format prior to access being provided.
Undertaking by media representative (Access Information) Form 3 PD9-2008 All media representatives requesting access to information and/or documents contained in a Tribunal file must sign an undertaking in this format prior to access being provided.
